Strong private hospitals in two cities. A thinner map everywhere else.
This is not medical advice. It is a description of how care is organized, so a visitor or a new resident knows where to start.
Foreign residents in Quito and Cuenca generally use private clinics. Hospital Metropolitano in Quito is a long-established private hospital. Cuenca has several private hospitals of its own. Quality in those two cities is the reason people name them in the cities chapter. Outside them, emergency care exists and specialist care means a drive.
Many doctors in private practice in the two cities speak some English. Do not count on it at a public pharmacy or a small-town clinic. Bring the generic names of your medicines.
Pharmacies are widespread. Fybeca is one of the national chains. Some drugs that require a prescription at home are easier to buy here, and some specialty drugs are simply not on the shelf. Carry a supply of anything you cannot interrupt, and a list.
Visitors should carry travel medical insurance that covers evacuation. Residents often use a local private plan, an international plan, or both. A residence visa typically asks for health insurance that is valid in Ecuador. That requirement is explained on the official visa pages linked from the visa chapter. Read the policy, not the slogan.
IESS is the public social-security system. Some foreign residents affiliate and use its hospitals and clinics. It is a real system, with waits and with coverage that can be broad. It is not a concierge practice. Rules for voluntary affiliation change. Confirm them with IESS if you plan to join.
Quito’s altitude is a health fact, not a lifestyle detail. People with heart or lung conditions should talk to their own doctor before a move, not after a listing.
